Smooth Criminal... how long has this plan been in the making?

I already knew about the Gilda reference in "This Is It", but I never really tied it back to "Smooth Criminal" before. I remember in an older interview Michael was talking about "Smooth Criminal" and how he always gets so many questions about it. He then said something to the effect of "I don't want to say to much about it". In Moonwalk, he is also very vague when discussing the meaning of the song. He says "Let's just say, it was the mood I was in." I've always suspected, even as a child, that the music video for Smooth Criminal had a secret meaning (the "nazi-like" police force, the mad scientist/dictator who wanted to have every child in the world hooked on drugs, the metaphors [ex: when he transforms into a car]). It really makes me think...how long has he been planning this? Did he discover something about the music industry and the government during the making of the "Bad" album? He seemed to always be afraid he didn't have very long to live. Why?

If this is he way of speaking out against something going on in the music industry and or the government then he may not have been helped by police dept. unless all of them have been paid off? Which brings me to another question, how big is this plan or rather how small is this plan? What is the minimum number of people to needed to pull it off?
